Please complete & return this form no later than March 31, 2024. All members must be in good standing to be included in the RVSPG Resource Guide. This form must be completed to apply for our annual Community Impact Grants. YOU ARE NOT A MEMBER OF RVSPG UNLESS THIS COMPLETED FORM HAS BEEN RECEIVED. 

Active membership in the River Valley Senior Provider Group entitles the member to attendance at meetings, networking opportunities, a free listing in the annual Resource Guide, member event notices, and a list of members. Each active member is entitled to one vote. You are encouraged to join the River Valley Senior Provider Group for the benefits associated with being a member in good standing. All members agree to follow Code of Ethics of the organization as outlined in Bylaws.
